Subcontractor Information
The following Subcontractor forms are available as downloadable files. All documents may be printed out and then mailed or faxed back to us when appropriate.

• W-9 for subcontractors who are working with Erland Construction for the first time.


• Subcontractor Qualification Statement for subcontractors who are interested in establishing a relationship with Erland. A W-9 must accompany this form.


• Application for Payment for subcontractors billing Erland for work performed.


• Application and Certificate for Payment - AIA Document G702
